Advance Directive

The Community Alliance developed an advance directive. This document is designed to promote education and conversation for individuals wishing to document their healthcare decisions for end of life. Included in the document is a durable for healthcare surrogacy. The Community Alliance’s Advance Directive has been recognized by The Office of the Missouri Attorney General in the Life Choices, March 2009 publication. This publication is free and available at the Community Alliance office or The Office of Attorney General in Jefferson City at    573-751-3321 . Through our Speakers Bureau, we educate and inform the community on the importance and need for everyone at every age to have an advance directive.

The Advance Directive November 2011 Revision is the only revision that Community Alliance supports. We no longer support any other unused versions. All past unused versions are to be discarded. If there are any past versions on websites or other communication mediums - those versions must be replaced immediately with the November 2011 Revision.
