An Advance Directive , also known as Health Care Directive or Living Will , is a document that allows you to state your wishes regarding the use of life prolonging medical treatment.
A Durable Power of Attorney for Health Care is a document that identifies who will speak as your agent (spokesperson) when you are unable to speak for yourself.
